Job summary

AVIR at Victoria in Victoria, TX seeks a compassionate Physical Therapist (PT) to join the in-house rehabilitation team. We offer Full-Time and Per Diem opportunities, with a focus on patient-centered care and collaboration within an interdisciplinary team to enhance residents' mobility and independence.

Responsibilities include evaluating residents, developing and updating PT plans, and educating patients and caregivers.

Qualifications

• Current Texas Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program.
• Strong clinical, communication, and organizational skills.
• Experience in skilled nursing or subacute rehab preferred.
• New graduates are welcome to apply.

• Evaluate residents and develop individualized physical therapy plans.
• Provide skilled PT services based on residents' needs and plan of care.
• Deliver therapeutic exercises, gait, balance, transfer, mobility, and functional activities.
• Monitor progress and adjust treatment plans as needed.
• Educate residents and caregivers on mobility, safety, exercises, and activities.
• Collaborate with PTAs, OTs, COTAs, SLPs, nursing staff, physicians, and other team members.
• Participate in care planning and communicate progress and needs.
• Complete accurate and timely clinical documentation.
• Maintain compliance with facility, state, and federal regulations.

AVIR at Victoria is seeking a compassionate and dedicated Physical Therapist (PT) to join our in-house rehabilitation team. We are hiring for both Full-Time and Per Diem opportunities.

This is an excellent opportunity for a Physical Therapist who enjoys providing patient-centered care and collaborating with an interdisciplinary team to help residents improve mobility, function, and independence.

Position Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.

• Provide skilled physical therapy services based on residents' needs, goals, and plan of care.

• Provide therapeutic exercises, gait training, balance activities, transfer training, mobility training, and functional activities.

• Monitor residents' progress and modify treatment plans as appropriate.

• Educate residents and caregivers regarding mobility, safety, therapeutic exercises, and functional activities.

• Collaborate with PTAs, OTs, COTAs, SLPs, nursing staff, physicians, and other members of the interdisciplinary team.

• Participate in care planning and communicate residents' progress and needs.

• Complete accurate and timely clinical documentation.

• Maintain compliance with facility, state, and federal regulations.
