Job summary

Healthfirst is seeking a Pharmacy Technician to act as a Pharmacy Navigator, reviewing medication lists, coordinating refills, and supporting MTM interventions to improve adherence. You will collaborate with pharmacists and external partners to ensure effective treatment plans and access to medications for members.

Qualifications

• Clear, concise, professional written and verbal communication skills.
• Experience communicating with patients, providers, and pharmacists on medication issues.
• Nationally certified Pharmacy Technician (PTCB, NHA) or registered/licensed Pharmacy Technician in good standing.

Responsibilities

• Reviews medication lists to identify barriers to therapy and promote adherence as Pharmacy Navigator.
• Facilitates access to medications by obtaining refills and new prescriptions, setting up appointments, and coordinating affordability.
• Communicates treatment plans with Healthfirst pharmacists and external partners to integrate MTM interventions.
• Monitors member medication adherence in coordination with community pharmacies.
• Conducts patient health assessments to identify barriers to adherence from medications and access to care.
• Handles member and provider communications telephonically; in-person as needed.
