Healthfirst is seeking a Pharmacy Navigator to review medication lists, facilitate refills and new prescriptions, and coordinate MTM interventions with internal and external partners.

The role emphasizes patient education, adherence support, and collaboration with community pharmacies to optimize therapy and reduce barriers to access. Strong communication and licensure as a Pharmacy Technician are required.

Qualifications

• High School Diploma or GED from an accredited institution.
• Experience communicating with patients, providers, and pharmacists on medication issues.
• Nationally certified Pharmacy Technician (PTCB/NHA) or licensed Pharmacy Technician in good standing.

Responsabilités

• Review medication lists to support adherence and safety as the member's Pharmacy Navigator.
• Assist members with refills, new prescriptions, appointments, and access to medications.
• Coordinate MTM interventions with Healthfirst pharmacists and external partners.
• Monitor member adherence with community pharmacies and communicate updates.
• Conduct patient health assessments and document medication-related information.
