Job summary

Haven Home Care Inc. seeks a Supervisory LPN to support our Eastern Ohio expansion on a per-visit basis. Compensation is per supervisory visit, open visit, skills test, and office time; mileage is reimbursed. There is no hourly wage and no guaranteed number of visits.

You will travel across Ashtabula, Columbiana, Mahoning and Trumbull counties, perform supervisory visits every 60 days, and assist with aide pre-employment testing. Flexible scheduling supports growth in this new territory.

Qualifications

• Must be at least 18 years old.
• Graduation from an accredited School of Practical Nursing.
• Current Ohio LPN license in good standing.
• Valid driver’s license, reliable transportation, and current auto insurance for work.
• Current CPR certification.
• Successful completion of criminal background check, skills test, and written nursing test.
• Residence in Trumbull or Mahoning County preferred.

Responsibilities

• Complete open visits and collect documentation per agency standards.
• Conduct supervisory visits every 60 days to check Home Health Aides.
• Perform pre-employment testing for Home Health Aide candidates.
