Job summary

Haven Home Care is seeking a Supervisory LPN to support our Eastern Ohio expansion. This per-visit role pays for supervisory visits, open visits, skills tests, and office time, with mileage reimbursement. There is no hourly wage and no guaranteed number of visits each week.

We are building our presence in Ashtabula, Columbiana, Mahoning, and Trumbull counties. Initial hours are minimal as we establish the client base, with visits increasing as clients come on board.

Qualifications

• Must be at least 18 years old.
• Current Ohio LPN license in good standing.
• Valid driver's license, reliable transportation, and current auto insurance that covers driving for work.
• Current CPR certification.
• Successful completion of criminal background check, skills test, and written nursing test.
• Residence in Trumbull or Mahoning County preferred.

Responsibilities

• Complete open visits and collect documentation following agency standards.
• Conduct supervisory visits every 60 days to check on Home Health Aides.
• Complete pre-employment testing for Home Health Aide candidates.

Education

Graduation from an accredited School of Practical Nursing
