Job summary

Haven Home Care is expanding in Eastern Ohio and seeks a Supervisory LPN to support our growth across Ashtabula, Columbiana, Mahoning and Trumbull counties. The role is per-visit, with visits and tasks billed individually, plus mileage reimbursements.

Initial hours may be limited as we build our client base from scratch in a brand-new territory. The LPN will perform supervisory visits, complete pre-employment testing for aides, and ensure accurate documentation while traveling to client homes.

Qualifications

• Must be at least 18 years old.
• Graduation from an accredited School of Practical Nursing.
• Current Ohio LPN license in good standing.
• Valid driver's license and reliable transportation.
• Current CPR certification.
• Successful completion of criminal background check and required tests.

Responsibilities

• Complete open visits and collect documentation per agency standards.
• Conduct supervisory visits every 60 days to Home Health Aides.
• Complete pre-employment testing (skills and written tests) for aides.
