Job summary

Karing Hearts Home Health is looking for a compassionate and skilled Per Diem RN to join our dedicated home health team in Vista, CA. Responsibilities:

• Conduct skilled nursing assessments and develop individualized patient care plans
• Administer medications, wound care, IV therapy, and other clinical treatments in the home setting
• Monitor and evaluate patient progress, adjusting care plans as needed
• Educate patients and family members on disease management, medications, and home safety
• Collaborate with physicians, therapists, and interdisciplinary team members to coordinate care
• Accurately document patient visits, clinical findings, and care plans in compliance with regulatory standards
• Ensure adherence to Medicare, Medi-Cal, and home health agency guidelines

Requirements:

• Active California Registered Nurse (RN) license in good standing
• Minimum 1 year of clinical nursing experience; home health experience preferred
• Current CPR/BLS certification
• Valid California driver's license and reliable transportation
• Familiarity with OASIS documentation and Medicare home health regulations is a plus
• Strong clinical assessment, critical thinking, and communication skills
• Compassionate, self-directed, and comfortable working independently in the field
