Job summary

Karing Hearts Home Health is seeking a compassionate Per Diem RN to join our home health team in National City, CA. This role offers one-on-one patient care in patients' homes and flexible scheduling.

You will conduct assessments, administer treatments, educate families, and coordinate with a multidisciplinary team to ensure quality care. Flexibility, strong communication, and reliable transportation are essential.

Qualifications

• Active California RN license in good standing.
• Minimum 1 year of clinical nursing experience; home health experience preferred.
• Current CPR/BLS certification.

Responsibilities

• Conduct skilled nursing assessments and develop individualized patient care plans.
• Administer medications, wound care, IV therapy, and other clinical treatments in the home setting.
• Monitor and evaluate patient progress, adjusting care plans as needed.
• Educate patients and family members on disease management, medications, and home safety.
• Coordinate care with physicians, therapists, and other interdisciplinary team members.
• Accurately document patient visits and clinical notes in compliance with Medicare/Medicaid guidelines.
• Respond to patient needs and changes in condition with timely clinical interventions.

• Strong communication, time management, and critical thinking skills
• Ability to work independently and compassionately with diverse patient populations
