Job summary

Karing Hearts Home Health is seeking a compassionate Per Diem RN to join our Santa Clarita team. The role focuses on delivering skilled nursing care in patients' homes with flexibility and autonomy.

Ideal candidates have an active CA RN license, at least 1 year of clinical experience (home health preferred), and strong knowledge of OASIS and Medicare guidelines. BLS/CPR and a valid CA driver's license are required.

Qualifications

• Active California Registered Nurse (RN) license in good standing.
• Minimum 1 year of clinical nursing experience; home health experience preferred.
• Current BLS/CPR certification required.
• Valid California driver's license and reliable personal transportation.
• Strong knowledge of OASIS documentation and Medicare home health guidelines.
• Excellent communication, time management, and organizational skills.
• Compassionate, self-motivated, and comfortable working independently in the field.

Responsibilities

• Conduct skilled nursing assessments and provide direct patient care in a home health setting.
• Develop, implement, and update individualized patient care plans.
• Administer medications, wound care, IV therapy, and other clinical treatments as ordered.
• Monitor and document patient progress, vital signs, and changes in condition.
• Educate patients and family members on disease management, medications, and care techniques.
• Collaborate with physicians, therapists, and interdisciplinary care team members.
• Ensure compliance with Medicare, Medicaid, and home health regulatory standards.
