Job summary

Karing Hearts Home Health in Escondido, CA seeks a compassionate Per Diem Registered Nurse to join our home health team. You will provide one-on-one care in patients' homes, perform skilled nursing assessments, wound care, IV therapy, and med management while coordinating with a multidisciplinary team.

Qualifications

• Active California RN license is required.

• Familiarity with OASIS docs and Medicare home health guidelines is a plus.
• Strong assessment, critical thinking, and communication skills.
• Ability to work independently in a field-based setting.

Responsibilities

• Conduct skilled nursing assessments and develop individualized care plans.
• Provide direct patient care including wound care, meds, and IV therapy.
• Monitor progress and adjust care plans as needed.
• Educate patients and families on disease management and home safety.
• Collaborate with physicians, therapists, and care team to coordinate services.
• Document assessments, interventions, and outcomes accurately per standards.
• Respond to patient needs per diem across Escondido service area.

Responsibilities:

• Conduct skilled nursing assessments and develop individualized patient care plans


• Provide direct patient care including wound care, medication management, and IV therapy


• Monitor and evaluate patient progress, adjusting care plans as needed


• Educate patients and family members on disease management, medications, and home safety


• Collaborate with physicians, therapists, and interdisciplinary team members to coordinate care


• Accurately document patient assessments, interventions, and outcomes in compliance with agency and regulatory standards


• Respond to patient needs on a per diem basis across the Escondido, CA service area

Requirements:

• Active and unrestricted California Registered Nurse (RN) license


• Minimum 1 year of clinical nursing experience; home health experience preferred


• Current CPR/BLS certification


• Valid California driver's license and reliable personal transportation


• Familiarity with OASIS documentation and Medicare home health guidelines is a plus


• Strong clinical assessment, critical thinking, and communication skills


• Self-motivated, organized, and able to work independently in a field-based setting
