Job summary

Karing Hearts Home Health in Del Mar, CA is seeking a compassionate Per Diem Registered Nurse to join our home health team. This role offers flexibility and the chance to positively impact patients in the comfort of their own homes.

As a nurse on our team, you will conduct assessments, provide skilled nursing care, educate patients and families, and work with an interdisciplinary team to coordinate care. The position supports independent field-based work with flexible scheduling.

Qualifications

• Active California RN license in good standing.
• Minimum 1 year of clinical nursing experience; home health experience preferred.
• Current CPR/BLS certification.
• Valid driver's license, reliable transportation, and active auto insurance.
• Strong knowledge of OASIS documentation and Medicare home health guidelines is a plus.
• Excellent communication, time management, and critical thinking skills.
• Ability to work independently with minimal supervision in a field-based environment.

Responsibilities

• Administer medications, wound care, IV therapy, and other clinical treatments in the home setting.
• Monitor and evaluate patient progress, adjusting care plans as needed.

• Accurately document patient visits, assessments, and interventions in compliance with agency and regulatory standards.
• Respond to patient needs and changes in condition with timely clinical judgment.
