Job summary

Karing Hearts Home Health in Burbank, CA is seeking a compassionate Per Diem RN to join our home health team. You will deliver personalized, one-on-one patient care in patients' homes, coordinating with the care team to ensure continuity and quality.

The ideal candidate holds an active CA RN license, CPR/BLS, and at least one year of clinical nursing experience, preferably in home health. Flexible schedule and supportive team culture make this a rewarding opportunity.

Qualifications

• Active California RN license.
• Minimum 1 year of clinical nursing experience; home health experience preferred.
• Current CPR/BLS certification.
• Valid California driver's license and reliable transportation.
• Familiarity with OASIS documentation and Medicare home health guidelines is a plus.
• Strong clinical assessment, communication, and time management skills.
• Compassionate, self-motivated, able to work independently in a field-based environment.

Responsibilities

• Conduct skilled nursing assessments and develop individualized patient care plans.
• Administer medications, wound care, IV therapy, and other clinical treatments in the home setting.
• Monitor and evaluate patient health status, reporting changes to the physician as needed.
• Educate patients and family members on disease management, medications, and care techniques.
• Collaborate with interdisciplinary care teams including therapists, social workers, and physicians.
• Maintain accurate and timely clinical documentation in compliance with Medicare and regulatory standards.
• Ensure patient safety and comfort while promoting independence and quality of life.
