Job summary

Karing Hearts Home Health is seeking a compassionate Per Diem Registered Nurse to join our Ventura, CA home health team. Deliver personalized, high-quality care in patients' homes with a flexible schedule that supports work-life balance.

The role includes skilled nursing assessments, medication administration, wound care, IV therapy, and coordination with the interdisciplinary team. A CA RN license, BLS/CPR, and reliable transportation are required; home health experience is preferred.

Qualifications

• Active California RN license in good standing.
• Minimum 1 year of clinical nursing experience; home health preferred.
• Current BLS/CPR certification.
• Valid California driver's license and reliable transportation.

Responsibilities

• Conduct skilled nursing assessments and develop individualized care plans.

• Monitor patient health, report changes to physician as needed.
• Educate patients and families on disease management and home safety.

Karing Hearts Home Health is looking for a compassionate and skilled Per Diem Registered Nurse to join our dedicated home health team in Ventura, CA. Responsibilities:

• Conduct skilled nursing assessments and develop individualized patient care plans
• Administer medications, wound care, IV therapy, and other clinical treatments in the home setting
• Monitor and evaluate patient health status, reporting changes to the physician as needed
• Educate patients and family members on disease management, medications, and home safety
• Coordinate care with interdisciplinary team members including therapists, aides, and physicians
• Accurately document patient visits, progress notes, and care plans in compliance with Medicare/Medicaid guidelines
• Ensure compliance with all state, federal, and agency home health regulations

Requirements:

• Active California Registered Nurse (RN) license in good standing
• Minimum 1 year of clinical nursing experience; home health experience preferred
• Current BLS/CPR certification
• Valid California driver's license and reliable transportation
• Familiarity with OASIS documentation and Medicare home health guidelines is a plus
• Strong communication, time management, and critical thinking skills
• Compassionate, self-motivated, and comfortable working independently in the field
