Job summary

Karing Hearts Home Health is seeking a compassionate Per Diem Registered Nurse to join our Lake Forest, CA home health team. You will deliver one-on-one, in-home patient care and collaborate with the care team to ensure patient comfort and safety.

Responsibilities include skilled nursing assessments, administering medications, wound care, IV therapy, and maintaining Medicare-compliant documentation. A California RN license and CPR/BLS certification are required, with at least 1 year of clinical

Qualifications

• Active California RN license in good standing
• Minimum 1 year of clinical nursing experience; home health preferred
• Current CPR/BLS certification required
• Valid California driver's license and reliable transportation
• Knowledge of Medicare home health guidelines and OASIS documentation a plus

• Conduct skilled nursing assessments and develop individualized patient care plans
• Administer medications, wound care, IV therapy, and other clinical treatments in the home setting
• Monitor and evaluate patient health status, reporting changes to physicians and care team members
• Educate patients and family members on disease management, medications, and care techniques
• Coordinate with interdisciplinary team including therapists, aides, and case managers
• Maintain accurate and timely clinical documentation in compliance with Medicare and regulatory standards
• Ensure patient safety and comfort while promoting independence and quality of life
