Job summary

Karing Hearts Home Health is seeking a compassionate Per Diem RN to provide one-on-one care in patients’ homes in Oxnard, CA. This role offers flexibility and requires strong clinical judgment, documentation accuracy, and the ability to work independently across the service area.

The ideal candidate will have an active CA RN license, 1+ year of nursing experience (home health preferred), CPR/BLS, and excellent IV skills to ensure high-quality patient outcomes in a dynamic home setting.

Qualifications

• Active California RN license in good standing.
• Minimum 1 year of clinical nursing experience; home health experience preferred.
• Knowledge of Medicare home health regulations and OASIS documentation.
• Current CPR/BLS certification.
• Valid California driver's license and reliable transportation.
• Strong clinical assessment, critical thinking, and communication skills.
• Ability to work independently and manage a flexible per diem schedule.
• Must have excellent IV skills.

Responsibilities

• Conduct skilled nursing assessments and develop individualized patient care plans
• Administer medications, wound care, IV therapy, and other clinical treatments in the home setting
• Monitor and evaluate patient progress, adjusting care plans as needed
• Educate patients and family members on disease management, medications, and home safety
• Coordinate care with physicians, therapists, and other members of the interdisciplinary team
• Accurately document patient assessments and interventions in compliance with Medicare and regulatory standards
• Respond to patient needs on a per diem basis across the Oxnard, CA service area
