Job summary

Karing Hearts Home Health in Carlsbad, CA is seeking a compassionate Per Diem Registered Nurse to join our home health team. You will bring skilled nursing care directly to patients in their homes, offering flexible scheduling and meaningful patient interactions.

You will perform assessments, administer medications, educate families, and coordinate with physicians and therapists, while documenting visits and ensuring regulatory compliance.

Qualifications

• Active California RN license in good standing.
• 1+ year of clinical nursing experience; home health preferred.
• Current CPR/BLS certification.
• Strong clinical assessment and communication skills.
• Compassionate, self-motivated, able to work independently in the field.

Responsibilities

• Conduct skilled nursing assessments and develop individualized patient care plans
• Administer medications, wound care, IV therapy, and other clinical treatments in the home setting
• Monitor and evaluate patient progress, adjusting care plans as needed
• Educate patients and family members on disease management, medications, and home safety
• Coordinate care with physicians, therapists, and other members of the interdisciplinary team
• Accurately document patient visits, clinical findings, and care delivered in compliance with regulatory standards
• Respond to patient needs and changes in condition with timely clinical intervention

About the Role:

Karing Hearts Home Health is looking for a compassionate and skilled Per Diem RN to join our home health team in Carlsbad, CA. Requirements:

• Familiarity with OASIS documentation and Medicare home health guidelines is a plus
• Strong clinical assessment, critical thinking, and communication skills
• Compassionate, self-motivated, and comfortable working independently in the field
