Job summary

Hartford HealthCare Organization is seeking an experienced Registered Nurse care coordinator to assess patients and develop discharge plans within an interdisciplinary team. You will address barriers to post-acute care and collaborate with providers to optimize hospital days and care transitions.

You will lead clinical teams to ensure daily goals align with the patient’s care plan, fostering seamless transitions and maintaining high-quality care across CT locations.

Qualifications

• Graduate from an accredited school of nursing.
• Previous experience in acute care setting.
• Valid Registered Nurse License in Connecticut.
• Case Management Certification preferred.
• Preferred bilingual English/Spanish.

Responsibilities

• Develop discharge planning within an interdisciplinary team.
• Identify and remove barriers to post-acute care access.
• Collaborate with providers to minimize delays and optimize hospital days.
• Provide clinical leadership to healthcare teams and ensure care plan goals are met.
