UnitedHealth Group is seeking a Per Diem Physical Therapist Assistant to support skilled care in patients' homes under a licensed PT. You will implement treatment plans, monitor progress, and collaborate with caregivers to enhance mobility and safety at home.

Qualifications

• Graduate from an accredited PTA program per CAPTE.
• Current license to practice as a PTA in the assigned state.
• Current CPR certification.
• 1+ years of PTA home health care experience or equivalent.
• If <1 year experience, mentoring or residency required.
• Driver's license and access to reliable transportation.

Responsibilities

• Provide physical therapy services under supervising PT per plan of care.
• Record patient responses and report changes to the PT and staff.
• Promote wellness and provide resources for future needs.
• Document supervisory activities in the patient record.
• Monitor patient response to therapy and adjust as needed.
• Participate in care conferences and quality improvement activities.
• Maintain patient confidentiality and coordinate referrals.
• Perform other duties as assigned.
