Job summary

UnitedHealth Group is seeking a Per Diem Physical Therapist Assistant to provide skilled PT services in patients’ homes under the supervision of a licensed Physical Therapist. You will implement treatment plans to improve mobility, strength, and independence while promoting safety and quality of life at home.

Requirements include a CAPTE-accredited PTA program, current PTA license in the state, CPR certification, and 1+ years of related PTA experience.

Qualifications

Graduate from an accredited physical therapy assistant program per CAPTE requirements.
Current license to practice as a PTA in the assigned state.
Current CPR certification.
1+ years of PTA experience in home health care or related field (mentoring possible for less experienced).

Responsibilities

Provide PT services and interventions under supervision of a licensed PT.
Observe and report patient responses and changes in condition to the PT and clinical staff.
Provide resources for future needs and wellness.
Document supervisory activities in the medical record.
Monitor and document patient response to PT interventions.
Participate in care conferences, inservices, audits and quality improvement activities.
Maintain confidentiality of patient information.
Coordinate follow-up care with the admitting agency and other care providers.
Perform other duties as assigned.
