ComplexCare Solutions is seeking a Nurse Practitioner to provide per diem in-home and telehealth assessments across Loudoun County, VA. The role emphasizes evaluating health status, identifying gaps in care, and delivering evidence-based care plans to keep patients in their homes and out of the hospital.

The NP must hold an active license, board certification (ANCC/AANP) in Family/Adult/Gerontology or Emergency Medicine, and CPR certification.

Qualifications

• Active un-encumbered NP license.
• ANCC or AANP board certification in Family, Adult, Gerontology or Emergency Medicine.
• CPR certification.
• Home health or remote assessments experience a plus.
• Bilingual or multilingual abilities are a plus.

Responsabilités

• Conduct comprehensive in-home and telehealth assessments.
• Document encounter findings and treatment plans.
• Coordinate with PCP and health plans; escalate urgent cases as needed.
• Maintain HIPAA compliance and patient confidentiality.
• Provide patient education to improve chronic condition knowledge.
• Maintain minimum 25 slots per month (~37 hours).
