Job summary

ComplexCare Solutions is seeking a Nurse Practitioner per diem to conduct comprehensive home and telehealth assessments for our health plan clients across Colorado and beyond. You will spend up to a full hour with members to capture their needs, document findings, and coordinate with PCPs for timely follow-up.

Remote work options enable flexible scheduling while maintaining high standards of care. The role requires ANCC/AANP certification, active nursing license, and knowledge of CMS

Qualifications

• CMS regulations and NCQA HEDIS guidelines familiarity.
• Knowledge of ADA/ACC/AHA practice standards and ICD-10/CPT-4 coding.
• Ability to review member data thoroughly and document encounters.
• Ensure comprehensive documentation compliant with CMS/DHHS and client policies.
• Communicate results and recommendations to PCPs and health plans.
• Close gaps in care and manage chronic conditions.

Responsibilities

• Review assigned member population comprehensively and submit encounter documentation.
• Document active medical conditions per CMS/DHHS and client requirements.
• Maintain timely communication with physicians, patients and health plans.
• Provide evidence-based treatment plans and follow-up timelines.
• Perform.Point of Care testing and document results.
• Ensure HIPAA compliance and company policies.
