ComplexCare Solutions is seeking a Nurse Practitioner Per Diem to perform home and virtual assessments for members in El Dorado County, CA. The role requires an active nursing license, ANCC or AANP certification, and the ability to work independently in a remote setting. Compensation ranges from $2,400 to $10,000 per month based on completed assessments. Benefits include a 401(k) plan. This role fosters diversity and promotes equity in healthcare delivery.

Formación

• Active un-encumbered license to practice nursing.
• ANCC or AANP board certification as a Nurse Practitioner.
• Maintains current CPR certification.
• Home Health experience is a plus.
• Ability to practice autonomously in a remote clinical environment.

Responsabilidades

• Complete comprehensive reviews of assigned member population.
• Document all pertinent medical conditions compliantly.
• Support physician/patient relationships through timely communication.
• Address identified gaps in care and perform Point of Care Testing.
• Maintain compliance with Company policies and HIPAA requirements.

Conocimientos

Effective communication with elderly patients

Excellent customer service skills

Ability to multitask

Compliance with healthcare personnel requirements

Understanding of Medicare/Medicaid

Bi-lingual or multi-lingual

Educación

Active un-encumbered nursing license
