Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in the Liberty, MO area. You will provide direct patient care, assessments, education, and care planning in home or alternate sites, following care matrices and best practices.

The role offers flexible scheduling, 10–20 hours per week, and travel within the service area. Must maintain an active RN license in KS and MO; BSN preferred and infusion experience encouraged.

Qualifications

• Current RN license in good standing (KS and MO).
• BSN preferred from an accredited program.
• Minimum of 3 years clinical nursing experience, infusion therapy preferred.

Responsibilities

• Provide direct care for infusion patients, initiating and maintaining communications with the care team.
• Educate patient/care partner to safely administer home infusion therapy and provide instruction.
• Evaluate home environment and plan safe, appropriate care.
• Develop and evaluate the plan of care with the team; monitor responses.
• Coordinate patient care using the approved platform and manage scheduling.
• Participate in nurse on-call rotation for continuous nursing support.
• Document patient encounters and care activities per policy and regulation.
• Maintain knowledge of Soleo therapies, protocols, and standards.
• Submit plans of treatment (POT) and required documents promptly.
