Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in the Independence, MO area. The role involves direct patient care, education, and coordination of home infusion therapy across the care team.

Responsibilities include documenting encounters, maintaining safety in home settings, and participating in on-call rotations. Requires active RN license in KS/MO and infusion experience; travel within service area is expected.

Qualifikationen

• Current Registered Nurse license in good standing with KS/MO Board of Nursing.
• BSN preferred from an accredited nursing program.
• Minimum of 3 years of clinical nursing experience, infusion therapy preferred.

Aufgaben

• Provide direct care for assigned patients including initiation and maintenance of communication with care team members.
• Educate patients and care partners to safely administer home infusion therapy.
• Assess home environment and ensure safety for prescribed care.
• Develop and evaluate plan of care with the team; adjust per response.
• Manage schedule via company platform and communicate changes promptly.
• Coordinate patient care using approved company platform.
• Participate in nurse on-call rotation to support continuous nursing services.
• Document all patient encounters and care activities per policy and regulatory requirements.
