Job summary

S Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in Grain Valley, MO and surrounding area. The role provides primary patient care during home infusion, including assessment, care planning and education, in accordance with care matrices and best practices.

Responsibilities include direct patient care, collaboration with pharmacists and prescribers, coordinating care, documenting encounters, and maintaining knowledge of Soleo therapies.

Qualifications

• RN license in good standing ( KS/MO )
• BSN preferred from accredited program
• Minimum 3 years clinical nursing experience w/ infusion therapy
• Demonstrated peripheral IV (PIV) insertion and vascular access skills
• Experience with home infusion care and patient education

Responsibilities

• Provide direct care for assigned patients and initiate/maintain communication with care team
• Educate patients and care partners on safe home infusion therapy
• Assess home environments and ensure safety for prescribed care
• Develop and evaluate plan of care with the team and document at time of service
• Coordinate patient care via approved company platform and schedule
• Participate in on-call rotation to support nursing services
