Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support our infusion patients in Platte City, MO and surrounding area. Provides primary patient care, assessments, care planning, education in home or alternate site.

Responsibilities include coordinating care with the care team, educating patients, documenting encounters, and on-call rotation. Travel within service area is required; mileage reimbursement applies.

Qualifications

• Current RN license in good standing with KS/MO Boards of Nursing.
• BSN preferred from an accredited program.
• Minimum of 3 years of clinical nursing experience, infusion preferred.
• Experience with peripheral IV (PIV) access and port access (PACs/Port-a-Caths).
• Infusion therapies including biologics, IV hydration; critical care backgrounds valued.

Responsibilities

• Provide direct care for assigned patients including initiation and maintenance of infusion therapy.
• Educate patients and care partners on safe home infusion administration.
• Assess home environments and plan contingencies for safe care.
• Develop and evaluate care plans with the care team.
• Coordinate care via approved platforms and document encounters timely.
• Participate in nurse on-call rotation to support 24/7 coverage.
• Maintain knowledge of Soleo therapies, protocols, and standards.
