Job summary

Ideal candidates have an active RN license, BLS certification, EMR experience, and a flexible schedule to accommodate hospice services.

Qualifications

• Current RN license in the state of employment.
• Active BLS for healthcare professionals (AHA/Red Cross).
• Minimum 1 year nursing experience in hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Valid driver’s license and automobile insurance per company policy.
• Reliable transportation to meet visit schedule.
• Ability to communicate effectively in English.

Responsibilities

• Assess and identify patient/family needs using the nursing process.
• Coordinate the Plan of Care with the Interdisciplinary Group (IDG).
• Provide palliative and supportive care to patients and families.
• Obtain physician orders and initiate skilled nursing interventions as needed.
• Document per policy and ensure safe discharge planning.
