Chapters Health System is seeking a Per Diem RN in Florida to provide palliative and supportive care. You will assess patient and family needs, plan individualized care with the IDG, and coordinate with multiple teams to ensure high-quality care.

The role requires an active Florida RN license, BLS certification, and at least one year of hospice or hospital nursing experience. A valid driver’s license and access to reliable transportation are essential for visits across multiple sites.

Qualifications

• Current RN license in the state where the employee will be working (Florida).
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Minimum one year nursing experience in hospice or hospital setting preferred.
• Experience working with an EMR/EHR system.
• Mobile driver with valid license and automobile insurance per policy.
• Reliable transportation to meet visit schedule.
• Ability to communicate in English (verbal and written).
• Flexible schedule to meet Hospice House patient needs.
• Ability to perform essential functions and physical requirements.

Responsibilities

• Provides and manages direct care to patients and families as part of IDG with psychosocial, spiritual, and physical components.
• Coordinates the Plan of Care reflecting patient’s evolving needs.
• Educates patient and family about disease, palliative care, and safety practices.
• Reports changes in condition to IDG and other health professionals.
• Attends patient deaths; prepares body for transport and oversees medication disposal.
• Presents concise reports to IDG and collaborates with other disciplines.
