Job summary

Chapters Health System is seeking a Per Diem RN to provide compassionate, patient- and family-centered hospice care within our facilities and home settings.

The role focuses on assessing needs, coordinating with the IDG, delivering quality palliative care, educating families, and ensuring smooth admissions and safe discharges while maintaining documentation and compliance with policy.

Qualifications

• Current RN license in the state where the employee will be working.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Minimum one year of in-patient nursing or hospice experience (external candidates); internal applicants: 6 months LPN with hospice/home health within affiliate.
• Experience with an EMR/EHR system.

Responsibilities

• Assess patient and family needs and coordinate Plan of Care with the IDG.
• Provide palliative and supportive care to patient and family.
• Ensure timely documentation and communication with physicians and IDG.
