Chapters Health is seeking a Per Diem RN to assess patient and family needs, coordinate the Plan of Care with the IDG, and provide palliative and supportive care across facilities in the service area. The role requires RN licensure in Florida, BLS, and strong communication skills.

Responsibilities include nursing assessments, educating families, documenting per policy, and coordinating with physicians to ensure timely and appropriate care.

Qualifikationen

• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Minimum one (1) year nursing experience in hospice or hospital experience preferred.
• Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system.
• Mobile Driver - Valid driver’s license and automobile insurance per Company policy.
• Reliable transportation to meet visit schedule.
• Ability to use equipment with visual and auditory mechanisms.
• Ability to effectively communicate in English (verbal and written).
• Flexible schedule to meet the needs of Hospice House patients.

Aufgaben

• Provides and manages direct care to patients and families as part of Interdisciplinary Group (IDG).
• Coordinates the Plan of Care with an individualized Plan of Care reflecting evolving needs.
• Educates patient/family about disease processes, palliative interventions and safety practices.
• Reports changes in patient condition to IDG or other health professionals.
• Attends patient deaths and prepares body for transport; oversees medication disposal.
