Job summary

Chapters Health System in Florida is seeking a Per Diem RN to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Group, and provide compassionate palliative and supportive care.

Requirements include RN license in the state, BLS, EMR experience, valid driver’s license and reliable transportation, and the ability to work flexible hours and handle emotional stress. Travel across service area is required.

Qualifications

Current RN license in the state of employment (Florida).
Active BLS for healthcare professionals.
Minimum 1 year hospice or hospital nursing experience preferred.
Experience with EMR/EHR systems and documentation.
Valid driver's license and automobile insurance; reliable transportation.
Ability to communicate effectively in English, both written and verbal.

Responsibilities

Provide direct patient and family care within the Interdisciplinary Group (IDG).
Coordinate Plan of Care reflecting patient needs with IDG.
Educate patients and families on disease, palliative care, and safety.
Report changes in condition to IDG and physicians; participate in admissions.
Ensure timely documentation and safe discharge planning; support bereavement needs.
