Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in Independence, KS and surrounding area. You will provide direct home care, coordinate with care teams, and educate patients for safe infusion therapy in the home.

Responsibilities include scheduling, on-call participation, and ensuring documentation compliance. Must have RN license, infusion experience, and ability to travel up to one hour (with potential two hours). Per diem hours support flexible weekly coverage.

Qualifications

• Current RN license in good standing with the state's board.
• RN degree (BSN) from an accredited school preferred.
• Advanced certification (CRNI, ONS, IgRN) preferred but not required.
• Minimum 3 years nursing with infusion therapy background preferred.
• ICU or cardiac care experience preferred.
• Proficient in Outlook and MS Office; familiarity with AlayaCare and similar platforms.

Responsibilities

• Provide direct patient care for infusion therapy in home or alternate site.
• Coordinate with care team, including pharmacists and prescribers.
• Educate patient and care partner on safe home infusion administration.
• Develop and evaluate care plans; document encounters per policy.
• Manage schedule using company platform; communicate changes promptly.
• Participate in nurse on-call rotation for continuous support.
• Ensure required documents (POTs, consents) are completed timely.
