Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in Independence, KS and surrounding areas. The role involves direct patient care, education, and coordination with the care team in home or alternate sites.

Responsibilities include assessing environments, developing care plans, and documenting encounters. Requires current RN license and infusion experience; BSN preferred. Per diem schedule ranges 10–20 hours per week with mileage reimbursement and competitive pay.

Qualifications

• Current RN license by the state’s Board of Nursing, in good standing.
• RN degree, BSN preferred.
• Advanced certification in specialty areas (e.g., CRNI, ONS, IgRN) preferred but not required.

Responsibilities

• Provide direct care for assigned patients and coordinate with care team including pharmacists, prescribers, dietitians, and home care partners.
• Educate patients and care partners to safely administer home infusion therapy.
• Assess home environments and identify safety interventions for prescribed care.
• Develop and evaluate the plan of care with team members.
• Manage schedule via company platform and communicate changes promptly to team members.
• Coordinate patient care via approved platform and participate in on-call rotations.
• Document all patient encounters and care activities per policy and regulatory requirements.
• Maintain knowledge of Soleo therapies, protocols, and clinical standards.
