Job summary

Senior Care Partners PACE seeks a hands-on leader to oversee daily home care operations and support a dedicated team of PCAs. You will collaborate with the IDT to deliver person-centered care, coordinate supplies, and ensure timely documentation in the EMR.

The role emphasizes field support, policy development, coaching, and payroll oversight, with a focus on compassionate service and team growth within a mission-driven organization.

Qualifications

• Minimum 2 years of professional leadership experience OR equivalent education and relevant work experience.
• CNA certification or willingness to complete PCA training provided by Senior Care Partners PACE
• At least 1 year of experience working with the elderly, frail, or long-term care populations.

Responsibilities

• Oversee PCA workflow to ensure high-quality participant care.
• Coordinate tasks, breaks, and breaks for PCAs and provide coverage as needed.
• Ensure care documentation is accurate in the EMR.
• Lead and develop the PCA team and payroll processes.
• Represent PCAs at IDT meetings and support care planning.
