Job summary

Habitat Health is seeking a PCA Supervisor to lead the Personal Care Attendant team at the PACE center, ensuring safe, dignified, high-quality ADL assistance. You will supervise daily workflows, provide training, and ensure regulatory compliance in coordination with the IDT, nursing leadership, and Center Director.

The role requires CNA certification, an associate degree or supervisory experience, strong communication, and a focus on quality and continuous improvement in a fast-paced healthcare

Qualifications

• CNA certification from accredited program.
• Associates degree or at least 2 years of supervisory, lead, or preceptor experience.
• Active Food Handlers Certification.
• Demonstrated knowledge of ADL support, infection control, and safe patient handling.
• Strong communication skills; interface with IDT, nursing, families, and participants.
• Experience overseeing daily workflows and supporting team performance through coaching and feedback.
• Ability to make routine operational decisions, solve problems in real time, and maintain quality standards.
• Demonstrates integrity, ownership and alignment with company values.
• Ability to thrive in a fast-paced, evolving environment with adaptability and curiosity.

Responsibilities

• Supervise PCA team daily assignments to ensure participant coverage at the day center and for external medical appointments.
• Serve as first point of escalation for PCA staff questions and daily operational issues.
• Ensure logs are documented timely and accurately per facility processes.
• Oversee PCA supply delivery and monitor center inventory.
• Ensure timely PCA staff documentation (ADL support, progress notes, grievances).
• Conduct regular check-ins, team huddles, and one-on-ones to support performance.
• Perform performance evaluations, coaching, and corrective action as needed.
• Coordinate ongoing competency evaluations and care plan alignment.
• Monitor ADL support quality against individualized care plans.
• Communicate status changes and care plan concerns to IDT and nursing leadership.
