Job summary

Senior Care Partners PACE is seeking a hands-on leader to oversee daily home care operations and support a dedicated PCA team within our PACE program. You will collaborate with the IDT to deliver high-quality, person-centered care for participants in the community.

In this role, you will mentor staff, manage payroll for direct reports, and help develop policies that strengthen home care delivery while maintaining compassionate, patient-centered service.

Qualifications

• Minimum 2 years of professional leadership experience OR equivalent education and relevant work experience.
• CNA certification or willingness to complete PCA training provided by Senior Care Partners PACE
• At least 1 year of experience working with the elderly, frail, or long-term care populations.

Responsibilities

• Lead daily home care operations and organize PCA workflow for high-quality participant care.
• Oversee scheduling, coverage, and field troubleshooting for PCAs.
• Coordinate supplies with the IDT and manage department inventory.
