Job summary

The Northeast Arc seeks a Skills Trainer to support the MassHealth PCA program, helping individuals with disabilities live independently. You will conduct home visits, provide hands-on education, and collaborate with nurses, therapists, and staff in a hybrid role across the community.

You will guide clients through PCA applications, payroll, and program management, building lasting relationships while maintaining thorough documentation and compliance with Medicaid requirements.

Qualifications

• High School Diploma or equivalent required; BA/BS in Human Service preferred.
• Experience with individuals with disabilities preferred; bilingual a plus.
• Strong interpersonal and communication skills; proficient in MS Office.

Responsibilities

• Assess the personal care needs of individuals in collaboration with a nurse and occupational therapist, and prepare PCA applications for submission to the consumer's insurer.
• Teach individuals how to hire, train, supervise, and pay their own Personal Care Attendants once program approval is received.
• Provide skills training to consumers with an active Prior Authorization, following the approved Medicaid curriculum, including how to be an employer and manage payroll.
• Conduct home visits to assess consumer needs and deliver in-person support and training.
• Maintain accurate and timely documentation of consumer interactions and progress in the program database.
• Support consumers and surrogates through ongoing case management and changes in care needs.
• Collaborate with PCM team to ensure coordinated service delivery and participate in department meetings.
