Job summary

The Northeast Arc is hiring a Skills Trainer to support the PCM program and help individuals with disabilities navigate the MassHealth PCA program. The role combines office and community-based work, delivering hands-on education and ongoing support to promote independence and home living.

Responsibilities include PCA applications, training clients to hire and manage PCAs, and coordinating with nurses, OT, and admin staff to ensure high-quality service delivery.

Qualifications

• High school diploma or equivalent required.
• Experience with individuals with disabilities preferred; bilingual a plus.
• Excellent communication and organizational skills.
• Proficient in MS Office and data entry.

Responsibilities

• Assess the personal care needs of individuals in collaboration with a nurse and OT, and prepare PCA applications.
• Teach individuals how to hire, train, supervise, and pay their PCAs.
• Provide PCA program education to consumers and guide them through Medicaid processes.
• Conduct home visits to assess needs and deliver in-person support and training.
• Maintain accurate documentation of consumer interactions in the program database.
• Support consumers and surrogates through ongoing case management.
• Collaborate with PCM team to ensure coordinated service delivery.
• Participate in department meetings and trainings related to the Medicaid PCA Program.
