Job summary

HCA Florida Memorial Hospital is seeking a Patient Logistics Specialist to coordinate intake of referrals and updates between Emergency Departments, inpatient settings, and community partners. You will respond to calls, faxes, and emails, gather information for clinical assessment, locate resources, and help with bed placement and transfers to support timely patient flow.

This full-time day role requires onsite work at an HCA Patient Logistics Center within 60 miles of Jacksonville, with weekend

Qualifications

• 2+ years of healthcare experience preferred.
• Behavioral Health experience preferred.
• Call Center – Transfer Center – Bed Management experience helpful.

Responsibilities

• Responds to phone calls urgently with strong customer service.
• Gathers initial information to open new case and provides to clinicians.
• Helps locate resources and coordinates information for disposition and admissions.
• Maintains bed availability inventory for non-HCA bed placements.
• Accepts, tracks and facilitates hospital transfers and transports via phone and computer.
• Provides updates to customers on status of patient cases and bed placement requests.
• Assists to schedule and prioritize discharge transports and support the Patient Logistics Center.
• Assesses patient acuity using critical thinking to facilitate patient flow.
