Job summary

HCA Healthcare is seeking a Patient Logistics Specialist to manage intake of new case referrals and coordinate with Emergency Departments, Inpatient Hospital Settings, and Community Settings. The role handles calls, faxes, and emails to obtain clinical information, admissions, or transfers.

The position involves prioritizing discharge transports, updating customers on case status, and ensuring rapid response to service requests.

Qualifications

• 2+ years of healthcare experience preferred.
• Behavioral Health experience preferred.
• Call Center/Transfer Center/Bed Management experience helpful.

Responsibilities

• Responds to phone calls with urgency and good customer service.
• Gathers initial information to open new case/call and prepares clinical information packets.
• Locates resources and coordinates information for patient disposition and admissions.
• Maintains active inventory of bed availability for non-HCA bed placements.
• Accepts, tracks and facilitates hospital transfers and transports telephonically and via computer.
• Provides updates on status of patient cases and bed placement requests.
• Schedules and prioritizes discharge transports and supports rapid response to service requests.
• Assesses situations using critical thinking to aid patient flow through care continuum.
