Job summary

HCA Florida Memorial Hospital is seeking an RN Coordinator for the Transfer Center in Jacksonville, responsible for coordinating patient transfers, placements, and movement across markets. The role emphasizes collaboration with facilities, physicians, and staff to optimize throughput and patient safety.

The candidate will apply nursing knowledge, clinical judgment, and strong communication to support efficient bed management and quality outcomes across the network.

Qualifications

• Graduate of an accredited nursing program; Bachelor’s degree preferred.
• Minimum of 3 years’ related clinical experience in an acute care setting (ED or Critical Care preferred).
• RN license in the states where the HCA facilities for that Division are located – multi-state license required if Transfer Center covers more than a single state (may be obtained upon hire)

Responsibilities

• Performs all transfer center functions, including centralized patient placement, in a timely, customer centered professional manner.
• Coordinates transfer of emergent, urgent and elective patients to a final destination facility from all potential referral sources.
• Facilitates patient throughput to a final destination facility with a focus on quality and efficiency.
• Works well with others and promotes teamwork by demonstration of good team member behaviors.
• Patient placement functions will ensure timely bed placement and accurate patient information for initial plan of care.
• Ensures accurate data collection and maintenance in patient records and center workflow systems.
• Demonstrates ability to triage and prioritize patient information for accepting physicians and hospital staff.
• Communicates professionally with referring facilities and physicians regarding all patient transfers and bed placements
