Responsibilities

• Schedule patient assessments and Oasis visits timely.
• Assign and schedule clinicians efficiently by geography and care needs.
• Coordinate with field staff and clinicians to complete visits as ordered.
• Handle scheduling-related calls from patients and staff.
• Refer issues to the clinical manager and monitor patient status.
• Communicate with patients, clinicians, referrals, and office teams.
• Generate reports and manage workflow actions.
• Assist with patient transfers between centers or services.
