• Schedules patient assessment visits including Oasis visits in a timely manner.
• Assigns clinicians to geography to maximize efficiency and match care needs.
• Collaborates with field clinicians to ensure all visits are completed as ordered.
• Handles calls from patients and staff regarding scheduling issues.
• Refs clinical issues to the clinical manager for guidance.
• Monitors hospitalized patients and ensures timely follow-ups.
• Communicates with patients, clinicians, referral sources, and office staff.
• Runs reports and responds to workflow with appropriate actions.
• Assists with transfers of patients between care centers and hospice services.
• Maintains supply closet and liaises with field staff for patient supply needs.
• Performs other duties as assigned.
