Qualifications

• 1 year scheduling experience in healthcare preferred.
• 1 year administrative experience in healthcare preferred.

Responsibilities

• Schedules patient assessment visits including Oasis visits.
• Assigns and schedules clinicians to maximize efficiency and match care needs.
• Coordinates with field clinicians to ensure visits are completed as ordered.
• Handles calls from patients and staff regarding scheduling.
• Escalates issues to clinical manager as needed.
• Monitors hospitalized patients and ensures necessary follow-ups.
• Communicates with patients, clinicians, referral sources, and other staff.
• Generates reports and acts on workflow.
• Assists with patient transfers between care centers and hospice services.
• Maintains supplies and liaises with field for patient supply needs.
