Job summary

Amedisys Home Health is seeking a patient services coordinator to schedule patient visits, coordinate clinicians, and manage communications across care centers. This on-site full-time role supports efficient patient care and seamless operations in a home health setting.

The position requires a high school diploma and at least one year of scheduling or administrative experience in healthcare, with opportunities for growth and comprehensive benefits.

Qualifications

• High School diploma or GED equivalent.
• One year of scheduling experience in a healthcare environment.
• One year of administrative experience in a health care environment.

Responsibilities

• Schedules patient assessment visits including Oasis visits in a timely manner.
• Assigns and schedules clinicians by geography to maximize efficiency.
• Coordinates with field clinicians to ensure visits are completed as ordered.
• Handles calls from patients and staff about scheduling.
• Escalates clinical or performance issues to the clinical manager.
• Monitors hospitalized patients and ensures follow-up communications.
• Generates reports and responds to workflow with corrective actions.
• May assist with patient transfer between centers and hospice services.
• Maintains supply closet and liaises with field for needs.
• Performs other duties as assigned.
