Qualifications

• High School diploma or GED required.
• At least one year scheduling experience in healthcare.
• One year administrative experience in healthcare with Microsoft Office.
• Strong customer service skills and ability to answer calls.

Responsibilities

• Ensure patient assessment visits and Oasis visits are scheduled and performed timely.
• Schedule clinicians efficiently by geography and required care needs.
• Collaborate with field clinicians to ensure visits are completed as ordered.
• Manage calls from patients and staff about scheduling issues.
• Refer issues to the clinical manager as needed.
• Monitor hospitalized patients and coordinate follow-up actions.
• Prepare and review reports and support workflow improvements.
• Assist with patient transfers between care centers or hospice services.
