Job summary

Amedisys is seeking a Patient Service Coordinator to support home health operations by coordinating services between patients, caregivers, clinicians, and office staff. The role emphasizes scheduling accuracy, timely communication, and operational efficiency.

The PSC role focuses on facilitating clinician assignments, managing visits, and ensuring smooth information flow across teams, with a strong emphasis on patient-centered care and reliable support for the clinical team.

Qualifications

• High school diploma or GED equivalent.
• 1+ year scheduling experience in a healthcare environment.
• 1+ year administrative experience in a healthcare environment.

Responsibilities

• Schedules patient assessment visits, Oasis visits, and clinician appointments.
• Coordinates clinicians across geography to maximize efficiency and match skill with required care.
• Communicates with patients, clinicians, referral sources and office staff to support care quality and efficiency.
• Manages scheduling-related calls from patients and field staff.
• Refers clinical and performance issues to the clinical manager.
• Monitors patient status and ensures timely follow-up as needed.
• Generates reports and acts on workflow findings; assists with patient transfers between care centers.
• Maintains supply closet and liaises with field for patient supply needs.
• Performs other duties as assigned.
