Responsibilities

• Manage inbound and outbound patient calls and messages professionally and compassionately.
• Qualify referrals and route to the appropriate team members for care coordination.
• Schedule patients for consultations, assessments, therapy, and other services as ordered by providers.
• Document calls, referrals, and scheduling in Salesforce and EMR systems accurately.
• Collaborate with clinical and administrative teams to ensure continuity of care.
