Job summary

Conifer Health Solutions, LLC. seeks a patient access/financial clearance specialist responsible for screening self-pay patients for eligibility in governmental and non-governmental programs.

You will identify payors, obtain and complete Confidential Financial Statement forms, and assist patients applying for benefits. Responsibilities include conducting patient interviews, documenting activities, and coordinating with hospital staff to ensure timely program enrollment and financial assistance

Qualifications

• High School diploma or equivalent.
• Minimum 2 years work experience with Social Services or Hospital Admitting or related area.

Responsibilities

• Conducts interviews with patients and/or family members.
• Records and maintains complete documentation of activities performed on account.
• Performs financial clearance including collections and assists with applying for benefits.
• Follows up on government program applications and communicates outcomes.
• Conducts field visits to assist patients with documents.
• Notifies hospital staff of case screening determinations and outcomes.
