Job summary

Conifer Health Solutions, LLC. is seeking a hospital-based Financial Counseling/Screening Specialist to support Abrazo Central.

The role focuses on screening self-pay patients for eligibility in government and non-government programs, determining payor sources, and guiding patients through Financial Assistance processes. Responsibilities include engaging patients and families, documenting activities, and coordinating with case management.

Qualifications

• Fluent English and Spanish preferred.
• Proficient with basic PC software and hospital systems.
• Experience in social services or hospital admitting preferred.

Responsibilities

• Screen self-pay patients for eligibility in governmental and non-governmental programs.
• Identify potential payors (auto insurance, Workers’ Comp, commercial/private insurance, TPL) to route accounts.
• Obtain and complete Confidential Financial Statement form; assist patients with applying for benefits.
• Record and maintain complete documentation of activities on account.
• Perform financial clearance including collections and prepare accounts for Financial Assistance review.
• Communicate screening outcomes to hospital staff and case management.
